• Completed application with signed and notarized affidavit.
• Application Fee: $100.
• Temporary permit: $25 (Good for 180 days).
• Verification of license via Nursys or using the Nursing Endorsement Verification Form.
• 2x2 passport style photograph (sign the front, bottom portion of the photo).

Licenses expire yearly on the 31st of October. You will need to complete an online renewal form or print and mail a renewal form. Submit your completed renewal form with a fee of $75.
